APD - Crew Custom Declarations The APD Declarations workbook provides template printing for the US Custom Declarations (CF 5129). The template pages print the crew information on CF5129 versions '96 and '01. A Generic and IMO full crew custom declaration list is available for use in foreign ports that accept this style. US Declaration: CF-5129 (02/01) The declaration template page is formatted to print crew information to the US Custom Crew Declaration form, CF-5129 (02/01).  Easy to use menu buttons quickly scroll through the crew list to display the individual crew members. US Declaration: CF-5129 (03/96) The declaration template page is formatted to print crew information to the previous version of the US Custom Crew Declaration form, CF-5129 (03/96).  Easy to use menu buttons quickly scroll through the crew list to display the individual crew members. IMO Crew Declaration Provides a standard IMO crew declaration for use in many foreign ports.
